Some of the sickest men I treat are not sick because of a disease. They are sick because something ended: a job, a marriage, a career, a role. I have reinvented my own career more than once, and I have watched men after 45 either rebuild or quietly fade. Purpose is often the difference.
Purpose is a health issue
In a study of nearly 7,000 U.S. adults over 50, published in JAMA Network Open in 2019, people with the lowest sense of purpose in life had more than twice the risk of dying during follow-up as those with the highest. The link was strongest for heart and circulatory deaths. That is an association, not proof, but it matches what I see at the bedside.
A five-step comeback plan
This comes from my book Success Reinvention, adapted for men starting over.
- Purpose. Write one sentence about who you want to help next. Your why leads to your what.
- Plan. Get the house in order first: money, health and the people who depend on you. Then name the obstacle most likely to stop you.
- People. Find a man who has done it, a man who will open a door, and a man you will teach. Nobody comes back alone.
- Skills. Learn one new skill this year, and practice it 20 minutes a day. AI tools are a smart place to start.
- Momentum. Take one visible step this week, then count it. Small wins are fuel.
Watch for the warning signs
A big loss can bring heavy drinking, poor sleep or thoughts of not wanting to be here. Those are not weakness. They are signals. If you or a friend are struggling, call or text 988. In danger right now, call 911.

Resilience and reinvention come up often in METal, the community for men founded by my friend Ken Rutkowski. You do not have to figure out the next chapter alone. Visit METal as my guest (my referral link).
I speak on this to men's groups, companies and conference stages. Book me to speak, or read more at https://www.HarveyCastroMD.com.
